Use the form below to order ITEL shipping supplies and forms. Supplies are sent to you free-of-charge, and since the labels pre-paid, you won't need to pay for shipping when sending the sample. For your convienence, the shipping fee for sending the sample to ITEL will be billed as part of your test fee.
