ITEL Invoices
If your company is being billed for the ITEL analysis, the invoice will appear at the bottom of the lab report.
The invoice section will itemize the charges and provide your Customer ID a Control Number to help you track your invoices. Please remember to include your Customer ID and control number when paying invoices.
Test Fee
The ITEL fee for the laboratory analysis will be listed on the invoice. Additional fees for optional testing will be itemized if applicable. Please contact ITEL Customer Service online or by calling 800-890-ITEL for current test fees.
Shipping
If a pre-paid ITEL shipping envelope and label (available online) are used for sending the sample, the applicable fee will be added.
Total Due
The total due reflects the shipping and test fees.
Control #
This is a unique number given to each ITEL test. For ease of billing, this number will be on both your invoice and the laboratory report.
Customer ID:
The Customer ID references your account with ITEL.
Federal Tax ID:
The Federal Tax ID number for ITEL is located on the bottom right corner of the invoice section. ITEL’s federal tax ID number is 59-3207296.
You may also receive the invoice and lab report on separate pages. Please write “separate invoice and report” in the comments section of the Test Request Form (download Word version) if you would like this option.
