For Homeowners: Sending a Sample to ITEL

Sending your flooring or siding to ITEL will help you and your adjuster find a fair settlement with our independent and unbiased analysis.

Test and Shipping Fee

The instructions below are for homeowners who have not been directed by their insurance carrier to send the sample as part of the claims settlement process. In this case, you will be paying for the analysis.

Step 1: Download the Form and Instructions

You can download the required Test Request Form, collection, submission, and price sheet in PDF format. There are separate downloads for flooring and siding:

Step 2: Print a Shipping Label

You can print a pre-addressed, pre-paid shipping label online through UPS by clicking the the button below. A new window will open to print the shipping label. Once you have filled in the return address and printed the label, close the window to return to this page.

Step 3: Collect and Send the Sample and Form

Carefully read and follow the sample collection instructions downloaded above. You can also view step-by-step instructions online at this link. When sending the sample, make sure to enclose a completed Test Request Form with the sample.

Submission Complete!

When using the pre-printed shipping label above, ITEL will receive your sample within two business days. Once ITEL receives your package, the sample is tested and results are sent to you on the next business day.
